How do you report a lost or stolen passport?
- Online is fastest: the State Department cancels the passport within 1 business day and emails a confirmation.
- By mail, fill out Form DS-64, then print, sign and mail it. This can take several weeks.
- Or report it in person when you apply for the replacement.
Only report a passport that is still valid. An expired passport cannot be used for travel anyway.
How do you replace it?
Reporting does not replace the passport. You apply in person on Form DS-11 and include where and when the loss or theft happened. Attach a police report if you have one.
Bring proof of citizenship such as a certified birth certificate, a valid photo ID and one passport photo.
Can you use it if you find it later?
No. A passport you reported lost or stolen stays canceled even if it turns up. Trying to travel on it can lead to delays or being denied entry to another country.
What if your trip is soon?
If you travel within 14 days, you need an appointment at a passport agency, and the nearest one to Arlington is in Dallas, or a private expediter who files for you.

Can I use my passport if I find it after reporting it lost?
No. Once reported, it is canceled and no longer valid for travel.
Can I renew instead of replacing a lost passport?
No. A lost or stolen passport is replaced in person on Form DS-11, not renewed by mail.
Do I need a police report?
It is not required, but the State Department asks you to attach one if you have it, along with where and when the loss happened.
